| Due to Federal regulation, it is imperative that we prohibit persons other than the patient from receiving communications (verbal or written) with regard to any part of medical care provided by our physicians and/or their staff. It will be necessary for the patient (or their legal guardian) to provide us with written notice of those individuals authorized to obtain access to patient information, either through verbal or written communications.
